Rutgers, The State University of New Jersey, is seeking a Community Coordinator for the Eric B. Chandler Health Center at the Robert Wood Johnson Medical School.
The Community Coordinator will be responsible for helping patients with
HIV and their families to navigate and access community services, other resources and adopt healthy behaviors. The position supports medical and behavioral health care providers through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain and improve the health of patients and their family.
Among the key duties of this position are the following:
- Provide services to patients living with HIV and their families.
- Conduct job activities in a clinical setting and in the community.
- Collaborate with staff to outreach patients living with HIV who are out of care and reconnect them to HIV medical and support services.
- Conduct outreach to the hardest-to-reach, at-risk communities including individuals who are substance-using, mentally ill, chronically incarcerated, uninsured, homeless, etc.
- Coordinate services with the clinical staff and medical case manager to help patients navigate resources that enhance wellness. Services include conducting eligibility determinations, making referrals, assisting with completing applications, helping patients access the resource, etc. Community resources may include food assistance, transportation, housing, legal aid, access to medications, specialty care and behavioral health services.
- Conduct treatment adherence counseling with patients. Provide accurate information to patients about living with HIV, preventing further transmission, the benefits/challenges of HIV care and treatment and available services.

Qualifications
| Minimum Education and Experience |
- High School Diploma or its equivalent.
- Experience working in a multi-cultural setting.

| Preferred Qualifications |
- Successful completion of a Community Health Worker formal training program from a college or other education institution is preferred.
- Shared life experiences with people living with HIV preferred.
- Experience working in a community-based setting and/or in the field of HIV for at least 1 to 2 years preferred.
- Knowledge of some medical terminology preferred.
