Under the direction from the supervising social worker, the incumbent will work with individuals living with HIV, those affected by HIV, and families to provide comprehensive social work services within an ambulatory care setting. These services include, but are not limited to: psychosocial assessments, assisting patients in referrals to emergency and long-term housing stabilization, substance use counseling, mental health assessments, develop and administer behavioral interventions, and making appropriate referrals to community agencies for financial benefits, legal, and immigration resources. The incumbent will provide a full array of social services that include, but are not limited to: crisis and behavioral interventions, provider consultations, insurance, and financial counseling, and participate in multi-disciplinary case conferences on an ongoing basis; in which care plans are developed for patients. The incumbent will be required to participate in social work meetings, staff meetings, case conferences, and other required Ward 86 Clinic meetings.

Ambulatory Care Social Services
Tracking healthcare utilization of patients, scheduling appointments, making appointment reminders, and following up on missed appointments. Performing psychosocial intakes and assessments for new and existing patients, including conducting comprehensive substance use and mental health assessments, formulating and administering interventions based on findings, and evaluating outcomes and of support systems and resources. Providing one-on-one counselling to patients for substance use, mental health, and overall wellbeing. Conducting de-escalation and crisis management as needed which may include conducting assessment and completing an involuntary hold (5150) and/or Tarasoff. Assessing patients for inter-personal or domestic violence, elder abuse, child abuse and making the appropriate referrals and police reports based on findings. Making ongoing assessment for child abuse and neglect and initiating referrals to Child Protective Services as appropriate. Participating in safety planning and crisis intervention - filing police reports, restraining orders, child protective orders in cases of active domestic violence. Assisting patients in accessing financial benefits and money management. Referring patients to community-based case management agencies/organizations, substance use programs residential and outpatient, mental health, psychiatry, and psychotherapy. Working with patients around transitional and permanent housing options through referrals to Transitions stabilization shelter, Hummingbird and stabilization rooms, HIV emergency stabilization rooms and permanent housing options through waitlists. - Providing comprehensive social services to patients with multiple complex psychosocial issues who present for care without a scheduled appointment. This may include providing assistance with navigating systems to access available benefits and resources to the community
Performing other general social work related duties as assigned.

Required Qualifications:
- Master's degree from an accredited school of social work, and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position
- Active license with the California Board of Behavioral Sciences as a Licensed Clinical Social Worker (LCSW), Associate Clinical Social Worker (ASW) or equivalent.
- Possession of the requirements necessary for application for licensure as a Licensed Clinical Social Worker as determined by the California Board of Behavioral Science Examiners; or equivalent
- Two (2) to four (4) years of social work experience providing services to HIV positive populations that are diverse in gender, sexual orientation, language, country of origin, race, and ethnicity.
- Ability to work in a high stress, high volume area and work well under pressure and with deadlines.
- Ability to handle demanding workload involving multiple tasks effectively and efficiently.
- Ability to function independently as a collaborative, supportive team member.
- Must demonstrate cultural humility and a strengths-based, recovery philosophy.
- Demonstrated excellent oral/telephone communication skills and written documentation.
- Must be computer-literate, working knowledge of HER systems, and Microsoft Suite.
