Social Worker (MSW) Outpatient Practice Make a Meaningful Difference in the Lives of Your Patients

Lee Health is seeking an experienced, outgoing, and compassionate Social Worker (MSW) to join our outpatient care team. This is an excellent opportunity for a confident professional who enjoys building relationships with patients and physicians, working independently, and helping patients navigate both healthcare and life challenges.

The ideal candidate is comfortable working with patients experiencing depression and other psychosocial challenges, has strong communication skills, and can confidently manage their own caseload as the sole MSW supporting the office.

What You'll Do

As the Social Worker, you'll serve as an important resource for patients, families, physicians, and the care team. Responsibilities include:

  • Conduct comprehensive needs assessments to identify patient and family needs

  • Provide case management and connect patients with appropriate community resources

  • Provide crisis management and intervention when needed

  • Support patients experiencing depression and other psychosocial challenges

  • Provide counseling, emotional support, and therapeutic interventions

  • Coordinate referrals and connect patients with appropriate healthcare and community services

  • Help patients navigate barriers affecting their health and overall well-being

  • Collaborate with physicians, psychologists, and other members of the interdisciplinary care team

  • Meet with physicians and introduce yourself as a resource for their patients

  • Assist with care coordination and interdisciplinary planning for high-risk populations

  • Conduct screenings and facilitate appropriate behavioral health resource connections

  • Advocate for patients and empower them to take an active role in their health and well-being

  • Provide support to patients and families adjusting to illness or changes in their health

  • Participate in coverage at another Lee Health office when circumstances require

What We're Looking For

We're looking for someone who is experienced, confident, personable, and comfortable taking initiative.

The ideal candidate will be:

  • Experienced in clinical social work, preferably within a medical or healthcare setting

  • Outgoing and approachable, with the ability to quickly establish rapport with patients and physicians

  • Comfortable working independently and managing responsibilities as the sole MSW in the office

  • Comfortable supporting patients experiencing depression and complex psychosocial needs

  • A strong communicator who can collaborate effectively with physicians, psychologists, and care teams

  • Organized and able to independently prioritize and manage multiple responsibilities

  • Compassionate, patient-focused, and confident in crisis situations

Requirements

Education

  • Masters Degree in Social Work (MSW) required

Experience

  • Minimum of 1 year of clinical experience in a medical or healthcare setting

  • Experience with case management, behavioral health, crisis intervention, or patient resource coordination is highly valued

License

  • License is not required for this position

  • Candidates interested in pursuing licensure may have the opportunity to discuss potential pathways and requirements
