Responsibilities
Position Summary:The Case Manager is responsible for coordinating safe, timely, and clinically appropriate discharge planning for patients receiving inpatient psychiatric treatment. Working collaboratively with patients, families, treatment teams, community providers, and external agencies, the Case Manager facilitates continuity of care by identifying discharge needs and barriers, coordinating referrals and aftercare services, arranging transportation, and ensuring all discharge planning activities are documented accurately and completed within established timeframes. This role requires proactive case coordination from admission through discharge to support successful patient transitions and positive treatment outcomes. Qualifications Education: Associate Degree required; Preferred - Bachelor's degree in Social Work, Psychology, Human Services, or a related field required. Experience: One to two years of case management, discharge planning, behavioral health, or healthcare experience preferred. Must demonstrate effective care of coordination, referral management, documentation, and collaboration with multidisciplinary teams and community providers.
