Position Summary:
We have an exciting opportunity to join our team as a Physician Assistant - Otolaryngology - Tracheostomy Care.

The Advanced Practice Provider (APP) will be a dedicated member of the Otolaryngology-Head and Neck Surgery (ENT) team, with a primary focus on the comprehensive management of patients with tracheostomies across both inpatient and outpatient settings.

This newly established role will provide clinical coverage for adult and pediatric ENT patients at Kimmel Pavilion (Kimmel) and Tisch Hospital/LOH, working collaboratively with the ENT physician team and existing APPs. The APP will partner closely with the pediatric ENT PA and adult ENT PA to provide coordinated, high-quality care across the ENT patient population.

The APP will serve as a clinical resource for tracheostomy management, education, care coordination, and clinical practice development, helping to establish and strengthen a standardized tracheostomy care model across ENT.

Primary Responsibilities Tracheostomy Management

* Provide comprehensive inpatient and outpatient management of patients with tracheostomies.

* Perform routine tracheostomy assessments and assist with tracheostomy changes, exchanges, and related procedures within the APP's scope of practice and institutional guidelines.

* Evaluate patients for tracheostomy-related complications, including obstruction, bleeding, infection, accidental decannulation, airway concerns, and other acute issues.

* Participate in tracheostomy initiation, ongoing management, and decannulation planning.

* Assess readiness for decannulation in collaboration with the ENT physician team and multidisciplinary care team.

* Provide post-tracheostomy and post-decannulation follow-up.

* Assist with management of complex airway and tracheostomy patients.

* Provide clinical recommendations regarding tracheostomy care and escalation of airway concerns.

Inpatient Responsibilities

* Perform daily assessments of ENT patients with tracheostomies.

* Collaborate with ENT surgeons, APPs, respiratory therapy, nursing, speech-language pathology, pulmonary/critical care, and other multidisciplinary teams.

* Participate in rounds and develop individualized plans of care.

* Respond to inpatient tracheostomy and airway-related concerns.

* Facilitate timely evaluation and management of tracheostomy complications.

* Assist with discharge planning and ensure patients and caregivers have appropriate education, supplies, follow-up, and home-care resources.

* Provide continuity of care when patients transition between ICU, inpatient units, rehabilitation, and home.

Outpatient Responsibilities

* Provide clinic follow-up for adult and pediatric tracheostomy patients.

* Perform focused airway and tracheostomy assessments.

* Assist with routine tracheostomy changes and other office-based procedures as appropriate.

* Monitor patients following tracheostomy placement, airway procedures, and decannulation.

* Identify patients requiring additional evaluation or escalation to the ENT physician.

* Provide ongoing education and support to patients and caregivers.

Pediatric and Adult Care

The APP will care for both pediatric and adult ENT patients, requiring the ability to adapt assessment, communication, education, and care plans to the needs of different patient populations.

The APP will work collaboratively with:

* Pediatric ENT PA, who provides dedicated pediatric ENT coverage.

* Adult ENT PA, who provides adult ENT coverage.

* ENT attending physicians, fellows, residents, nursing, respiratory therapy, speech-language pathology, and other multidisciplinary partners.

The APP will complement the existing ENT APP structure while providing specialized expertise and continuity for patients with tracheostomies.

Patient and Family Education

* Provide comprehensive education to patients, families, and caregivers regarding tracheostomy care.

* Teach routine tracheostomy care, suctioning, emergency management, signs and symptoms requiring medical attention, and appropriate use of supplies.

* Reinforce safe tracheostomy practices prior to discharge.

* Support families throughout the transition from hospital to home.

* Serve as an accessible clinical resource for tracheostomy-related questions and concerns.

Care Coordination

* Coordinate care among ENT, nursing, respiratory therapy, speech-language pathology, pulmonary/critical care, case management, home-care agencies, durable medical equipment providers, and other services.

* Promote seamless transitions between inpatient and outpatient care.

* Assist with ensuring appropriate equipment, supplies, home services, and follow-up are arranged prior to discharge.

* Facilitate communication between inpatient and outpatient providers to promote continuity of care.

Program Development and Quality Improvement

As this is a newly established role, the APP will have an important opportunity to help develop and advance the ENT tracheostomy program.

Responsibilities may include:

* Help develop and standardize tracheostomy care pathways and clinical workflows.

* Identify opportunities to improve patient safety, quality, efficiency, and continuity of care.

* Develop and maintain tracheostomy education resources for patients, families, and staff.

* Participate in quality improvement and evidence-based practice initiatives.

* Assist with development and implementation of standardized protocols, order sets, and clinical guidelines.

* Track and evaluate tracheostomy-related outcomes and opportunities for improvement.

* Identify gaps in care and develop solutions in collaboration with the ENT team.

* Participate in staff education and interdisciplinary training related to tracheostomy management.

* Contribute to the growth and long-term development of a coordinated ENT tracheostomy service across Kimmel and LOH.

Role Within the ENT APP Team

The APP will function as an integral member of the ENT APP team and will work collaboratively rather than as a separate service.

The APP will:

* Provide specialized tracheostomy expertise to support the entire ENT team.

* Collaborate with the pediatric and adult ENT PAs to ensure appropriate coverage and continuity.

* Provide cross-coverage and clinical support when appropriate.

* Serve as a resource to other ENT APPs and clinical staff for complex tracheostomy-related issues.

* Participate in ENT team meetings, education, quality improvement, and service initiatives.

* Promote consistent standards of care across adult and pediatric ENT populations and across both hospital locations.

* Help create a cohesive APP model that supports the overall goals and needs of the ENT service.

Minimum Qualifications:

* Master's degree from an accredited Nurse Practitioner or Physician Assistant program.

* Current NYS licensure and appropriate national certification.

* BLS certification; additional certifications as required by institutional policy.

* Clinical experience in ENT, airway management, critical care, surgery, pediatrics, or a related specialty preferred.

* Demonstrated ability to work effectively within a multidisciplinary team.

* Strong clinical assessment, communication, organization, and problem-solving skills.

* Ability to manage both adult and pediatric patients or demonstrated willingness and ability to develop competency in both populations.

* Interest in tracheostomy and complex airway management strongly preferred.

Required Licenses: Physician Assistant Lic-NYS, Ntl Com Cert Physician Asst, Basic Life Support Cert

Preferred Qualifications:
Advanced Life Support certification (ACLS, PALS) through the American Heart Association or Neonatal Resuscitation certification (NRP) through the American Academy of Pediatrics may be required based on your selected privileges.
Membership in pertinent professional organizations preferred.
