We are searching for a talented Advanced Practice Practitioner to join our Health at Home - Supportive Care Hospice - Palliative Consulting Team. Under direction of the Director of Supportive Care Services or designee, work in collaboration with a Physician, and other palliative care team members, in an active practice to deliver hospice care and related health care services to patients and families. Perform a wide variety of duties including but not limited to bedside patient/family evaluation, assisting with diagnosis, treatment and management of acute and chronic health conditions, educating/counseling patients, families, and/or caregivers as to preventative care, medical problems, etc, and serving as a nursing resource for education and consultation to members of the interdisciplinary team. Requires a Masters of Science Degree in Nursing. Certification from a national accrediting body to practice as a Nurse Practitioner is required. Current RN license in the Commonwealth of Massachusetts & Rhode Island is required. Minimum of 5 years nursing experience with clinical nursing care in oncology, hospice, geriatrics and/or intensive care preferred. A minimum of one-year Nurse Practitioner experience. Certification in Hospice and Palliative Care required. Must possess current CPR certification Certification from a national accrediting body to practice as a Nurse Practitioner is required. Current RN license in the Commonwealth of Massachusetts is required. Certification in Hospice and Palliative Care required. Must possess current CPR certification.
