Advanced Practice Provider Heart Failure ICU

Seeking an Advanced Practice Provider for the Heart Failure ICU team at UH Cleveland Medical Center. Join our dedicated Physicians and APPs as they provide a team-based collaborative approach to patient care. Responsible for the management of heart failure patients, heart transplants, temporary and durable mechanical circulatory devices including but not limited to IABP, Impella and LVAD's. The schedule will rotate days/nights (night shift differential), Weekend and Holidays. Nurse Practitioners must be Acute Care. Will consider new grads!

A Brief Overview

The Advanced Practice Provider is responsible for providing quality care for patients and increasing patient access and satisfaction. Standard tasks to be performed by an Advanced Practice Provider as specified by the Ohio State Board of Medicine or Nursing and UH entity specific Medical Bylaws.

• Contributes to the educational development of staff, patients, family and self (5%)
• Demonstrates progressive leadership and effective management (5%)
• Provides education through role modeling and in-services to meet identified learning needs and to promote professionalism (5%)
• Advances own professional development through self-directed learning, continuing education and participation in peer review (5%)

• Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
