NaphCare is hiring experienced PRN Social Services Case Manager just like you at the Middlesex County Jail located in North Brunswick, New Jersey. Pay rates vary based on shift, site needs, and experience. Shifts and hours vary based on coverage needs and can be discussed upon interview. PRN shifts and hours vary based on coverage needs for FT shifts and will be determined by site leadership and prn employee availability.

Responsibilities

Responsibilities for Social Services Case Manager:

  • Evaluate patients in a variety of settings, including the initial intake area, mental health care unit, medical unit, emergency situations, housing units, and segregation units.
  • Coordinate with healthcare and security staff regarding patients' release dates and ensure an effective discharge summary plan is documented within the patient's health record.
  • Assist patients with the identification of a support network, instructions for future care and follow-up, and concerns, complaints, and requests regarding care upon discharge.
  • Assist patients with completion of all clinical and financial documentation needed for social service organizations.
  • Meet with patients to discuss placement options, organize records for transition, and provide advice regarding community assistance organization, transportation options, etc.
  • When notified of discharge date, initiate involvement with patient and staff members as appropriate to formulate a discharge plan and treatment.
  • Involve patient in discharge planning, including the patient's level of functioning, financial resources, willingness to learn, and motivation to care for self.
  • Coordinate care with community agencies, shelters, employment agencies, drug and alcohol counselors, or other social service organizations.


Qualifications

Qualifications for Social Services Case Manager:

  • Bachelor's degree in a human service or mental health field
  • Valid CPR card
